New Patient Registration

The doctors welcome new patients who live within our practice area. 

We are extremely happy to receive new patient registrations from patients living within the practice boundaries, which lie between Barnstaple and Bideford Bridges, and approximately three miles inland from the coastline within this area. This includes postcodes starting with EX39 4, EX31 2 & EX31 3.

  

We believe that health care is best delivered by having one GP responsible for an individual patient’s care and every patient registered with the practice has a usual doctor. It will not always be possible for you to see your usual doctor, but he or she is your first option when requesting a consultation.

 

Please pop into reception and you will be given a registration pack to take away and complete including registration form, new patient questionnaire and application for online services. Once your registration pack is completed and returned to the practice we will arrange a convenient appointment for your new patient medical.

 

Alternatively you may print out copies of the registration documents below, fill them in and bring them in with you on your first visit to the practice.  Please note that when you bring in your completed registration form and questionnaire, you will need to bring with you some form of ID to prove your identity, preferably a passport, driving license or birth certificate.
