New Patient Registration
The doctors welcome new patients who live within our practice area.
We are extremely happy to receive new patient registrations from patients living within the practice boundaries, which lie between Barnstaple and Bideford Bridges, and approximately three miles inland from the coastline within this area. This includes most postcodes starting with EX39 4, EX31 2, EX31 3 & EX32 0. Please contact the practice if you are located on the edge of our catchment and are unsure if your postcode is included in our area.
We believe that health care is best delivered by having one GP responsible for an individual patient’s care and every patient registered with the practice has a usual doctor. It will not always be possible for you to see your usual doctor, but he or she is your first option when requesting a consultation.
Please pop into reception and you will be given a registration pack to take away and complete including registration form, NHS data sharing consent form, new patient questionnaire and application for online services. Once your registration pack is completed and returned to the practice we will arrange a convenient appointment for your new patient medical, and should you currently receive repeat medication, an appointment with the Clinical Pharmacist based at the practice.
Alternatively you may print out copies of the registration documents below, fill them in and bring them in with you to the practice to register. Please note that when you bring in your completed registration forms, you will need to bring with you some form of photo ID to prove your identity, preferably a passport or driving license and for children a birth certificate.
